Before you begin

I understand that, as a current member, I am completing this form to satisfy the State of Florida membership authorization requirement.

Step 1: Complete and sign the State of Florida Membership Authorization Form.

This form confirms your membership authorization. It does not replace any existing membership record or require you to create a new FEA membership account.

You only need to complete this form once. No further action is required unless EASL contacts you.

I UNDERSTAND THIS IS A STATE OF FLORIDA MEMBERSHIP AUTHORIZATION REQUIREMENT.
